For referrals to the clinic
Referrals can be made directly through Cerner. Providers who do not use Cerner should complete the clinic referral form. Please contact our clinic team to obtain the form.
Please include the following with your referral:
- Clinic notes or consultations
- Relevant laboratory results
- Echocardiogram, PYP scan, MRI, other relevant imaging
